Do You Have “Cortisol Face”? Understanding the Concept

For those of you who are frequently online and worried about your appearance, you might have recently scrutinized before-and-after photos, searching for signs of “cortisol face.” Characterized by a round, puffy, and bloated appearance, is this just another social media myth or does it have some truth to it?

First, it’s important to understand that cortisol is actually beneficial. This hormone is produced by the body in response to stress and plays a crucial role in regulating blood sugar, blood pressure, and inflammation. Online “experts” often claim that a puffy “cortisol face” results from elevated cortisol levels due to stress. It’s also referred to as “moon face,” though neither term is an official medical diagnosis.

While it’s true that improper cortisol production can lead to health issues, conditions like Cushing syndrome—characterized by excessive cortisol—are rare and typically accompanied by chronic high cortisol levels, which can indeed result in a rounded face.

In most cases, occasional puffiness or facial rounding is likely linked to less serious causes. When you’re stressed, elevated cortisol can lead to water and salt retention, impacting facial tissues. Additionally, medications such as steroids can cause fluid retention and bloating. Stress may also weaken the immune system, increasing susceptibility to inflammation, but this is generally seen in individuals with persistently high cortisol levels.

Everyday stressors in your life are unlikely to cause significant facial changes. If any alterations occur, they are typically subtle and temporary, unless you are experiencing chronic stress. To minimize puffiness, focus on staying well-hydrated and reducing your salt intake.
